Vocational Rehabilitation
Application For Services

If you are interested in applying for vocational rehabilitation services to help you get a job, please follow these steps.

1. Print the application form.
2. Answer the questions on the application form.
3. Review the Handbook of Services.
4. Contact your nearest SRS office to schedule an appointment with a Vocational Rehabilitation (VR) counselor. Or call toll-free 1-888-369-4777 and ask to speak to a VR counselor. (Individuals with hearing or speech impairments may also contact us through the Relay Center, toll-free 1-800-766-3777.)
