Rehabilitation Services Name
Transition Planning
Transition planning services are available to help special education students with severe disabilities plan for a smooth transition from school to employment. This program is a cooperative effort with Rehabilitation Services, special education programs, employers, adult service agencies, community rehabilitation programs, and residential organizations.

Through this program, rehabilitation counseling, information, and referral to community services are available for special education students who are at least 16 years old.

During the transition planning process, students and their parents have the opportunity to discuss a variety of issues with the counselor, including:

  • goals, priorities and preferences for life after school;
  • residential, employment and support programs;
  • application procedures, eligibility criteria or waiting lists for needed services;
  • information about community resources and assistive technology;
  • information about the Individual Education Plan (IEP) process, and the statement of needed transition services which is a required part of the IEP; and
  • advocacy.

Please contact Rehabilitation Services at your local SRS office if you need more information about transition planning services.
