- Child Care
- Child Support Services
- Medical Assistance
- Vocational Rehabilitation Services
- Energy Assistance
You can have someone else apply for you if you would like. You can also have someone help you fill out the application. If you have a guardian or conservator, that person must apply for you. You can apply for other family members who live with you. You can also apply for a family member who lives outside of your home. For example, you may apply for your parent who lives in a nursing home.
When you click the GO ON button below, you will go to the online application Login Page. The application takes about 30 to 45 minutes to finish.
Before you start, you may want to have the following information handy to help you complete this application:
- Social Security numbers for everyone you are applying for.
- Income information such as wages, child support, Social Security, etc.
- Resource information such as bank and savings accounts, vehicles, homes, land, life insurance, etc.
- Monthly household expenses such as rent, utilities, etc.
- Health insurance information.
- Social Security or Veteran's claim information.
- Child support court order.
If you can't fill out all of the questions and need to come back and finish later, make sure you click the QUIT SAVE INFO button at the bottom of the screen you are on. The work you have done will be saved. You can finish where you left off when you come back. You have up to 30 days to finish the application from the date you started.
You will have a chance to review all of the information that you enter and make changes before you submit the application.

SRS will be glad to help you with the voter registration application.
