Here's how it works. The SRS Online Self-Assessment asks questions about the household in order to determine services you may qualify for. You need to enter correct information about yourself and those who live with you in order for us to screen correctly. The information that is entered on the self-assessment form will not be saved by SRS. If you are applying for someone else and you do not want assistance yourself, only enter information for the person you are applying for.
At the end of the assessment, you can apply for cash assistance, food assistance, child care services, child support services, healthcare coverage, and vocational rehabilitation services. The self-assessment can only tell you what your household might be able to qualify for and let you know whether it would be worth your time to apply. If the self-assessment results say you may qualify, it is not a promise that you will get benefits and services. There are lots of other steps that must happen before you actually get benefits and services. You must submit an application for services in order for us to determine if you qualify for benefits and services.
If your browser does not support scripting, you may not be able to complete these forms online. As an alternative, you may want to access a computer at your local library. Or please contact your local SRS service center. If you would be charged for this call, you may call toll free at 1-888-369-4777.
