| Answer |
| No, unless there are some changes you see that need to be reported. You will need to have a copy of what was reported. With that |
| copy, you can check for any changes that need to be submitted for the next quarter. |
| Question |
| We used to have Supportive Employment, but discontinued the program with Voc Rehab. However, the people left in maintenance, |
| we left in day service, but fund with county mill. These individuals work in the community and attendance is not taken. How do I |
| answer the attendance question for these individuals? |
| Answer |
| What service is funded? If Day Service is funded, the agency must be doing comething for the person. Therfore, some should have |
| an idea of how many days the person works. |
| Question |
| Do I enter people in BASIS who are only receiving case management? |
| Answer |
| Yes, as long as they meet the criteria for inclusion in the system (see introduction in the BASIS manual). |

| If a person is receiving services in one CDDO's area, but wants to transfer to our CDDO's area, how do we include this person in our |
| BASIS data? |
| Answer |
| You don't. The person is served. If he or she wants to move to your area, DD Reform requires portability of funding. That is, his or |
| her funding can follow the person to your CDDO area. Your CDDO may want to track individuals internally, but should not include |
| them as waiting in your BASIS data. |

| Do the reporting agency and the case management provider have to match? |
| Answer |
| Yes, they are to be the same. The reporting agency is the agency providing the case management not the CDDO. If they are not in |
| your provider list, please fax to Linda Johnston (785-296-0494) the provider name and the effective date. |
| Question |
| For a child under 5 what sections of BASIS do you need to complete? |
| Answer |
| For a child under 5 you need to complete all sections accept the assessment section. The assessment needs to be completed in |
| the birth month when the child turns 5. |

| When we start entering the new sections of BASIS does it matter if they are totally complete when they start getting transmitted. I |
| am going to enter attendance and wages from affiliates or CDDO but I don't have all my information yet. So is it okay for my case |
| managers to enter what they have? Of course, the goal would be that everything would be answered and completed by February 15. |

| Questions to the psychotropic medications, PRN medications and systems analysis sections of BASIS can be updated |
| incrementally. Care will need to be taken to insure that all the data be put into the appropriate sections. Each time data is added to |
| each section the report date needs to be changed to indicate new material has been added. |

| What if I enter the wrong Social Security number? |
| Answer |
| You need to close the wrong number out. For the reason closing use "wrong SS#". Once you have closed it, you may enter the client |
| with the correct social security number. When completing the new assessment use wrong SS# for the assessment type. |
