Do you change the date in the Assessment when you change the type of report?

Answer

No, you do not change the date.

Question

Do we have to complete the Assessment section of BASIS on everyone we are serving?

Answer

If they are 5 years of age, and meet the MR/DD definition, you must complete all sections annually, regardless of how their services

are funded. You also must complete all sections for anyone 5 years of age who is waiting for service.

Question

What type of report is marked for individuals transferring from one CDDO to another? Does the new CDDO enter the Assessment as

an Initial even though it was existing at the originating CDDO?

Answer

No. You should mark it as Transfer from another Facility. You may not change any of the data in the fields from what the sending

CDDO had, unless it is time for a Re-evaluation.

Question

I submitted an initial assessment for an April birth date but the assessment date was keyed in as 04/01/99 when it should have been

04/01/00. How do we correct the assessment date?

Answer

If you catch the error prior to the cut off date for processing that month's data, you can change the date and change the evaluation

type to Annual and resubmit. It should be fine. If the processing cut off for that month is passed, you will need to get special

permission from CSS to change the assessment date, then you will need to change assessment type to Special Permission by

MH/DD..

When do we use Initial Assessment?

Answer

Initial Assessment should only be used the first time the person has an assessment. It does not matter who the original CDDO was.

On the next assessment, you would use either Readmit to the same facility or Transfer from another facility.

In the Assessment Section, Medical Conditions - indicate yes or no for each of the following: Respiratory, Cardiovascular,

Gastro-Intestingal, Genito-Urinary, Neoplastic Disease and Neurological Diseases. Is Cerebral Palsy considered a Neurological

Answer

No, Cerebral Palsy is not a neurological disease. It is a disabling condition which should be listed in #18 on the Information

Question

If a client receives dialysis twice a week (or at all) does this need to be marked as yes on (5d) Receive Medication by Injection?



If a client has a Baclophen pump does this need to be marked as yes on (5d)? A Baclophen pump is a pump about the size of a

walkman. It is inserted just underneath the surface of the skin in the abdominal area. This pump releases medications on a time

release. It has to be refilled every 3 months and a new one has to be inserted every 4-5 years.

Answer

No. Both of these should be captured in the medical consequences section - Question 6.
