Medicaid
If the provider is NOT already a licensed Alcohol and Drug Treatment Provider with SRS/AAPS, that process must be initiated and a provisional issued before Medicaid approval will be granted.

Computer System Requirements
In order to use the state AAPS system, these forms must be completed prior to receiving any state funding.
