STATE RESOURCES

4th Time DUI
Entities such as licensed substance abuse treatment providers; community mental health centers that provide substance abuse treatment; Indian Tribes and tribal organizations, etc. Applicants must demonstrate their ability and capacity to provide services to the identified priority populations.

Medicaid
If the provider is NOT already a licensed Alcohol and Drug Treatment Provider with SRS/AAPS, that process must be initiated and a provisional issued before Medicaid approval will be granted.

Computer System Requirements
In order to use the state AAPS system, this requirement form must be completed prior to any 4th Time DUI or Medicaid funding.
